You do not need a lawyer to do this. Don't wait for a lawyer. The living will for example is good even on a piece of napkin as long as your writing can be identified. Your will will be honored. That is the spirit of the documents. Don't be overwhelmed with the formalities. Write in a Word document and have your signature notarized.

Every state's health and human services website has those prescribed forms available. I know Texas forms are available here [hhs.texas.gov] A simple Google search will take you to your state's health department web pages.
