It depends on the skill of the movie, but in general I agree with you. The best way to terrify is by whipping plausible monstrosities into a frenzy, or any way of forcing sharp and jarring twists on the psyche. The best scare I ever had was The Shining (at age 14, with two days home alone ahead of me, in a new home in a new state, in winter). For all it's aspects, the foundation of that movies scare is the distinct possibility of a real human psychological break like that.
