It used to be that way for me. I could not have an orgasm unless I was manually stimulated, and even then it was difficult. I believe it was a shame thing. Parents of the Silent Generation, my parents, raised their children on guilt and shame. And sex was wrong, bad, dirty. Sex was an obligation that a woman owed to her husband.
