More likely to be anorexia on the right and poverty on the left . The impoverished , are more likely to buy cheaper food - high in starch , rather than a diet well balanced with a balance of vegetables and meat . While we think of the rich as being fat , they are less likely to be .

Could be body dysmorphia. Something than can affect literally anyone regardless of social status or any other factor.
Have to give the models credit for doing this.

I, with all my heart, I hope this was photoshopped. I know that this could be very real, as there are places in the world (USA) where poverty is this real.
@dalefvictor I don't see it as being about poverty. I see it as being about illness and image issues.

I had a cousin that spent most of her life battling anorexia. She was not as skinny as the pic above, but she would drive me crazy with her behavior.
Always talk about food, insist in buying food and then not eating and leaving you eating alone that junk food.
