Someone on Reddit posted something like this and one guy commented that his doctor was late and he billed the doctor's office the same penalty fee they charge late patients. The matter went through small claims court and the business eventually had to pay him the fee. Then they dropped him as a client.

I actually did this to a back specialist. I gave the bill to the receptionist more as a joke / making a point. The next time I had an appointment the doctor had the bill on my chart. He asked me about it and I explained the wait I had and that it cost me money to be away from work. He thought it over and said my bill seems fair and paid it.

I really wanted to do this yesterday.
3pm appt. They tell you to get there at 2:30. I didn't get called back to
an exam room until almost 4pm. By the time they were done with all
the details, I didn't get out of there until 4:30.
Didn't even see the doctor, just his PA.
I was SO pissed!

@Sheannutt I'm really lucky my nephew happened to be off and the kids are out of school for summer. Had this happened a month ago, he'd have been at work, and the kids would have still been in school.
I'd have missed bus drop off and one of their parents would have had to have left work early to be at home. I would have raised ten kinds of hell if that had been the situation.
