What a terrible horrible tragedy.
Wow! Very powerful. These are among the most difficult news stories to hear, and as he alluded, we often are quick to blame, criticize, and condemn the responsible party. Of course, there have been instances of pure neglect with these deaths, some involving substance abuse; but, I believe him when he points out how the distractions of everyday life, and the rigidness of routine can lead to these tragedies. How many times has any one of us gone into another room to get or do something, gotten in the room, and completely forgot why we went in there? It's not a greatly dissimilar phenomenon. This is why I try to practice mindfulness, which absolutely works for me. I know, because when I slip, and begin to let my mind be distracted by a hundred different concerns, things begin falling apart.
