For that to happen human actions would have to be subject to totally objective rules of morality, taught and enforced from birth with no room for maneuver in any circumstances whatsoever.
Since moral absolutes do not exist and all behaviour is subjective, responsibility and accountability becomes relative to circumstance.
One person may fully feel they have taken responsibility for an action while another person may feel that the same subject has unjustifiably totally abnegated themselves from all accountability
