When I was in high school in AZ, one JV basketball coach had a Navajo nickname but I never could pronounce it properly -- there are sounds in Navajo that don't exist in English.
He was really proud of it until someone told him what it meant -- 'has the balls of a baby turkey'.

It wasn't intended as nasty as Trump's name likely would be. The coach's nickname was pretty much just stupid high school kid clumsy humour. When you operate at certain levels, I guess that's what you get.
I'd think Trump's jab would probably be more serious.
