You don't provide information on the purpose of the survey, how the results will be used, where and when to find the results etc. Makes it look amateurish. Discourages participation from those who expect a higher level of integrity.
I'm not Canadian, but you're soliciting responses in the UK Atheists and Agnostics group and one of the questions asks if I'm Canadian, yet the survey is titled "Religious Beliefs of Canadians" this is a discrepancy likely to cause other non Canadians to baulk at the survey.
There's not much data in the survey that would help you weight the survey to balance the results across many types of demographic.
You ask if I'm agnostic or atheist and make that that an either or question. As it happens I'm both and not allowing me to answer as such makes you look ignorant of what those terms actually mean.
I answered I'm not sure on a couple of questions. Not because I have any doubt about my beliefs, but rather because you're using vague terms without clear definitions. I don't believe in a supernatural soul but not everyone who uses that term is referring to something supernatural.
Similarly as someone who's read up a little on eastern philosophies I probably have a different understanding of the word Karma from the typical interpretation. I don't believe in either supernatural interpretation: whether an essential quality being passed on through reincarnation or a divine retribution of misdeeds. I do however believe in a totally secular understanding that bad behaviour tends to be repaid by society as a whole at a greater rate than certain sociopaths might assume.
