Big Pharma mostly. As for myself I take 3 prescriptions as a generic and my usual cost is zero. My ace in the hole is my Veterans card where currently most medications regardless of price can be purcahsed for $15. If I move on into real insulin shots later I might have to start using this VA card.

When Bush Jr. was president, congress made it illegal for Americans to get their prescriptions filled in Canada and Mexico. The reason was blatantly obvious but when asked why, George made the excuse "We don't want their drugs to kill y'a" (Insert stupid smirk here). The same people that believed him are now known as trump supporters.

How sad it all is.

being the majority of our lawmakers are beholding to the drug manufacturers for large campaign contributions this is why we pay more for drugs in the US one does not bite the hand that feeds it.
