When "The Little Mermaid" the Disney cartoon first came out, on video, the cover had a design that incorporated penis shapes into it. It was the result of an employee who knew he was getting fired soon. They recalled them, and the original video covers are now collectors items... of course so are the VHS tapes inside the cover.
