I think this might be a flower stalk since it sprouted a sister plant off the back underside. I don't know what this succulent (cactus, it does have clusters of tiny needles) is so not quite sure. fingers crossed

How exciting that your plant is going to blossum. The plant appears to be a variety of Haworthia. Perhaps the best known that resembles your plant is called Zebra Plant, although I'm not certain this is what yours is. I have seen Haworthia varieties for sale with a flower stalk starting like your plant.

I have a small cactus just like this one. It has a habit of sending long sprouts from all parts (including the very middle) with a flower on top. The spider plant does the same thing.
