By the third time you recognize it during the mistake. By the fourth time you recognize it before the mistake, but go "fuck it" and do it anyway.

Some people maker the same mistake repeatedly and never learn from it. I once fired a teacher who had been teaching for 18 years. After close observation of his instruction, I could see that he was making mistakes that a beginning teacher should not make. He did not have 18 years of experience, but one year of experience 18 times.

That is a paraphrase and I don’t know who said it!
