Ipecac is a herbal syrup. It is what it is. There could be a different kind of extract or formula or drug that induces vomiting, that works better. But currently, it's considered bad practice to u dude vomiting when you don't know what you're doing, I actually thought that you couldn't buy ipecac any more.

when they give prisoners their lethal injections for execution WHY do they worry about alcohol swabs to clean the site before injecting it?

Since it is a "natural" substance, and designed to induce vomiting, which it does Very well, I think not.
